The rule

How is the cost report due date calculated?

A Medicare cost report is due on or before the last day of the fifth month after your cost reporting period ends. If your period does not end on the last day of a month, the report is due 150 days after the period ends (42 CFR 413.24(f)(2)). The calculator applies that rule, including leap years.

  • Fiscal year ending December 31: due May 31
  • Fiscal year ending March 31: due August 31
  • Fiscal year ending June 30: due November 30
  • Fiscal year ending September 30: due February 28 (February 29 in a leap year)
  • Period ending on any other day, such as June 15: due 150 days later (November 12)

Hospices only

Does the hospice aggregate cap have a separate deadline?

Yes. The hospice aggregate cap is a separate filing from the cost report, with its own due date. Your hospice files its self-determined aggregate cap determination with its MAC no later than 5 months after the cap year ends, and remits any overpayment. The cap year runs October 1 to September 30 (42 CFR 418.3 and 418.308(c)).

For the cap year ending September 30, 2026, that is February 28, 2027. Confirm the date with your MAC.

Why the date matters

What happens if a cost report is filed late?

If a provider does not timely file an acceptable cost report, Medicare payments can be suspended in whole or in part until an acceptable report is filed and accepted by the MAC (42 CFR 405.371). The same suspension rule applies to a late hospice cap filing.

Extensions are not routine. The MAC may grant one only when your operations are significantly adversely affected by extraordinary circumstances outside your control, such as a flood or fire.

When is a Medicare cost report due?

On or before the last day of the fifth month after the cost reporting period ends. If the period does not end on the last day of a month, it is due 150 days after the period ends (42 CFR 413.24(f)(2)).

When is the cost report due for a December 31 fiscal year end?

May 31 of the following year. For a fiscal year ending December 31, 2026, the report is due May 31, 2027.

When is the cost report due for a June 30 fiscal year end?

November 30 of the same year. For a fiscal year ending June 30, 2026, the report is due November 30, 2026.

What if my fiscal year does not end on the last day of a month?

Then the report is due 150 days after the period ends. For example, a period ending June 15, 2026 is due November 12, 2026.

Does a leap year change the due date?

Only for September 30 year ends. A fiscal year ending September 30, 2027 is due February 29, 2028, because the fifth month after it ends is February and 2028 is a leap year. In other years it is due February 28.

Can I get an extension on my cost report?

Only in narrow cases. The MAC may grant an extension when your operations are significantly adversely affected by extraordinary circumstances outside your control, such as a flood or fire. Routine requests are not provided for.

What happens if I file my cost report late?

Medicare payments can be suspended in whole or in part until an acceptable cost report is filed and accepted by your MAC (42 CFR 405.371).

Is the hospice aggregate cap due with the cost report?

No. It is a separate filing. A hospice files its self-determined aggregate cap determination with its MAC no later than 5 months after the cap year ends (October 1 to September 30), per 42 CFR 418.308(c). Confirm the date with your MAC.

What if my due date falls on a weekend or holiday?

Confirm the filing date with your MAC. The calculator shows the date the rule produces and flags weekend dates, but it does not apply any holiday adjustment.

Does this calculator cover Illinois IDOA cost reports?

No. It covers Medicare home health and hospice cost reports. The Illinois IDOA cost report follows a different rule: it is due within six months after the end of the reporting period (89 Ill. Adm. Code 240.2020(a)).
